What this policy covers

This policy explains how Haven Psychiatry & Wellness handles information collected through this website. It is separate from our HIPAA Notice of Privacy Practices, which governs your protected health information as a patient of the practice and is provided to you at your first visit and available on request.

Information we collect

We collect information you choose to give us — for example your name, phone number, email address, and anything you write in a message when you contact us or request an appointment.

Like most websites, we also receive basic technical information automatically, such as your browser type, device type, approximate region, the pages you view, and the site that referred you. This is used to keep the site working and to understand which pages are useful.

Appointment scheduling and the patient portal

Online scheduling and the patient portal are provided by CharmHealth, a third-party electronic health record platform. When you book an appointment or log in to the portal, the information you enter is submitted to CharmHealth and handled under their platform's security and privacy terms and applicable HIPAA requirements — not stored by this website.

We do not track, record, or transmit the provider, service, or appointment details you select inside the scheduling window to any advertising or analytics platform.

How we use your information

To respond to your questions, schedule and manage appointments, provide care, maintain and improve the website, and meet legal and professional obligations. We do not sell your personal information.

Sharing

We share information only with service providers that help us operate the practice and the website (for example our electronic health record, scheduling, email, and hosting providers), when you ask us to, or when required by law. Protected health information is shared as described in our HIPAA Notice of Privacy Practices.

Email, text, and messages sent through this site

Email and web forms are not fully secure channels. Please do not use them to send detailed health information. For anything clinical, call the office at 813-567-5675 or use the patient portal, which is designed for secure messaging.

Cookies and analytics

This site uses only what is needed to function and to measure general usage. Most browsers let you block or delete cookies in their settings. If advertising or analytics tools are added in the future, they will be configured so that they do not capture information about the treatments or providers you select while booking.

Your choices

You can ask us what information we hold about you, ask us to correct it, or ask us to stop contacting you, by emailing admin@myhavenwellness.com or calling 813-567-5675. Requests concerning your medical record are handled under HIPAA and Florida law.

Children

This website is not directed to children, and we do not knowingly collect information from children through it. We do treat minors as patients, with care arranged by a parent or legal guardian.

We may update this policy as the practice or the website changes. The date above reflects the most recent update.
